Job Description

A local nonprofit healthcare provider has an immediate opening for a Service Coordinator.


This full-time role will be responsible for interacting with members, providers, and physicians to coordinate care services and support to individuals with special health needs.  


Qualified candidates need to be passionate about serving members in the community and have relevant experience. The company offers generous benefits and has office locations in Hilo and Kona! 


Additional Responsibilities:


  • Conduct face-to-face health and functional assessments for members on an annual or more frequent basis as applicable
  • Facilitate member and provider authorization and access to services
  • Monitor and resolve any concerns about service delivery or providers and ensure that provided services are meeting the member’s needs
  • Provide individualized education on preventative health care measures
  • Maintain accurate written documentation and records of service coordination activities in computer system 
  • Ensure compliance with all state and federal regulations, including HIPAA standards of confidentiality of protected health information
  • Perform other duties as assigned 


Required Education and Qualifications:


  • Previous experience in healthcare administration, nursing, public health, and Medicare/Medicaid
  • Licensed Social Worker (LSW) or Licensed Clinical Social Worker (LCSW) in the State of Hawaii
  • TB Clearance and CPR Certification
  • Intermediate computer skills and experience using Microsoft Office
  • Passionate about serving members in the community 
  • Must be organized and detail-oriented
  • Strong integrity working with sensitive and confidential information
  • Valid driver's license
  • Bilingual in any of the following languages preferred: Ilocano, Tagalog, Mandarin Chinese, Japanese, Korean, or American Sign Language
