Position Summary

• With the goal to empower the patient and family through education and support, the Medical Social Worker (LCSW) provides services utilizing principles of palliative care and the hospice philosophy. Provides direct patient/family counseling and referrals to appropriate community resources as needed. Assists the patient and family in developing and enhancing optimal coping strategies. Supports the hospice philosophy and team approach to provide, with other members of the team, holistic care to the patient and family.

Duties & Responsibilities

• As indicated by the social work assessment and plan of care, provides social work services, including but not limited to: psychosocial assessment, counseling, consultation, education, resource and referral services, advocacy, and discharge planning.
• Participates in developing the interdisciplinary care plan both as a consultant and as a provider of direct services. Participates in the weekly interdisciplinary team conferences.
• Responsible for the initial bereavement assessment and coordinates bereavement plan of care.

Skill & Knowledge Requirements

• Ability to exercise independent sound judgment in planning and providing quality psychosocial care.
• Demonstrated application of critical assessment and thinking skills.
• Interest and desire to provide hospice care with a special sensitivity to the needs of seriously ill patients.
• Maintains adequate education and experience level to meet the needs of patients, care givers, and regulations established by State and Federal laws.

Educational and Experience Requirements

• Graduate of a school accredited by the Council of Social Work Education, with a minimum of a Masters degree in social work.
• LCSW license required for this position.
• Hospice & Palliative care experience preferred.

Hours

Exempt position. Scheduled, staggered weekdays and weekends, normally 40 hours/week with occasional evening and holiday hours.
