Full-Time Position
Position Summary:
Under the supervision and in collaboration with the Medical Director or designee, the Nurse Practitioner provides care services to patients and families in a variety of settings within the community. Responsible for performing comprehensive medical and psychosocial evaluations. Performs clinical care of potential and current patients and families faced with life-threatening or progressive, life-limiting health conditions. Includes administrative oversight and management of the outpatient clinic.
Duties & Responsibilities:
- Performs patient evaluations in a variety of settings within the community.
- As credentialed and privileged to perform, physical examinations, histories, chart documentation, patient orders, radiological and laboratory interpretation, discharge orders, patient & family instruction and education, and participation in case management.
- Provides documentation of clinical findings as a “result encounter” to the palliative care physician who is considering palliative treatment options and plans of care.
- Provides services related to palliative, and end of life care, assesses patient needs, and anticipates service requirements prior to discharge or change of place of service.
Skill & Knowledge Requirements:
- Full understanding and compliance with State of Hawaii and federal laws and regulations on nursing and nurse practitioner scope of work; responsibilities, professional principles, procedures and techniques of nurse practitioners as applicable within specific licensure.
- Advanced knowledge of symptoms and normal course of acute and chronic illnesses, and their interventions/treatments.
- Ability to chart on a laptop in preparing concise, legible, timely clinical notes, according to agency guidelines.
- Ability to work independently and apply sound medical judgment and hospice protocols.
Education & Experience Requirements:
- Licensed State of Hawai’i Nurse Practitioner (NP) in a specialty area that permits NP to diagnose, treat and prescribe to adult patients ages 18 and older (APRN RX).
- Valid State of Hawai’i RN license and a Masters in Nursing (MSN) from an accredited Nurse Practitioner Program required.
- Hold and maintain unrestricted license for full prescribing authority based on state and federal requirements
- Credentialed and privileged with health plans and facilities as required to perform duties and meet agency needs
- Valid CPR certification
- Must be fully vaccinated against COVID-19
Hours
- Exempt position
- Primarily Monday through Friday, 8:00 a.m. to 5:00 p.m., with occasional evening and weekend work as needed to accomplish the goals and objectives of the position. May be required to participate in on-call coverage with Hospice Physicians.
