Reports to: Director of Outreach and Interim Case Manager
The Qualified Mental Health Professional (QMHP) at HOPE Services Hawaii plays a pivotal role in delivering compassionate and effective therapeutic services to a diverse client population. The position requires the QMHP to provide behavioral health services to a transdisciplinary team in the professional delivery of services and works to manage crises and mitigate risk for people experiencing unsheltered homelessness T The QMHP works closely with the Director of Clinical Operations to ensure that therapeutic interventions and program services are meeting the needs of the unsheltered participants. The incumbent collaborates within a multidisciplinary team to address the mental health needs of individuals and contributes to the overall well-being of the community.
Essential Functions:
• Conduct individual and group therapy sessions employing evidence-based therapeutic modalities (must attempt 20 therapeutic sessions a week; these sessions may not align with traditional timeframes but must prioritize trust and rapport building, and promote therapy with individuals and/or families).
• Administer and complete various assessments to evaluate the mental health and well-being of clients and link them to appropriate services.
• Using a person-centered approach, collaborate with clients to develop and implement treatment plans tailored to their unique needs.
• Oversee and Manage Internal referral systems.
• Provide crisis intervention and support as needed.
• Stay abreast of current developments in the field of mental health and homeless services and integrate best practices into the therapeutic approach.
• Maintain accurate and up-to-date client records in accordance with ethical and legal standards.
• Support team members to ensure that programming is grounded in Housing First practices, harm reduction, motivational interviewing, and trauma informed care.
• Prepare findings, diagnostic impressions, diagnosis, and recommendations. Present information; written and verbal, to team members, participants and supervisors for the purpose of caseload management and/or case review.
• Provide case reviews to address difficult cases to process and help staff problem-solve around case management challenges, assessments, housing and discharge plans.
• Support teams in ongoing evaluation of the progress of program participants and adjust treatment plans in accordance with participant's capabilities, needs and interests .
• Provide feedback for and implement program policies and procedures.
• Conduct chart audits to ensure that all documentation meets standards of funders and agency policies.
• Contribute to reports and compile data for funders and agency leadership as required.
• Participate in agency meetings and training as required.
• Document all billable activities accurately in the electronic health record system, when applicable.
• Collaborate with other team members, including housing navigators and Team Leaders to enhance the overall effectiveness of therapeutic services.
• Assist and lead appropriate training for direct care staff.
• Participate in ongoing professional development and training.
• Perform other duties as assigned by the Director of Clinical Operations.
Work Environment:
This role operates in community-based settings and on the streets .. Temperatures may vary as working in the field, and the community includes varying weather - rain, wind, cold, hot, and humid conditions. Frequent exposure to stressful situations related to persons served includes but is not limited to domestic violence, sex-offenders, substance abuse, and people suffering from mental health issues. This role routinely uses standard office equipment such as computers, phones, photocopiers, filing cabinets, and fax machines.
Communication Demands:
Strong communicator with strong written and interpersonal communication. Able to communicate with audiences of varying comprehension levels, including staff, guests, and partnering agencies. Handle sensitive and confidential information with tact and professionalism.
Physical Demands:
The position requires adaptability, the ability to work under pressure, and strong communication skills to resolve conflict and foster collaboration across teams. The physical demands described here represent those that an employee must meet to perform the essential functions of this job successfully. While performing the duties of this job, the employee is regularly required to talk and hear. The employee is frequently required to stand, walk, and use hands to reach. Occasionally lift, bend, stoop, push, and pull up to 20 pounds.

- Preferred candidates have experience working with individuals experiencing homelessness with Severe Mental Illness, Substance Use, and Trauma
Required Education and Experience
- Licensure as a Qualified Mental Health Professional (e.g., LCSW, LMHC, LMFT, Psychologist) in the State of Hawaii
- Proven experience understanding and demonstrated ability to generate revenues for clinical services
- Working knowledge of trauma-informed care principles and harm reduction interventions. Comfortable using and supervising staff in motivational interviewing.
- Demonstrated ability to establish and maintain rapport with program participants, other staff, and outside agency representatives.
- Ability to communicate effectively verbally and in writing.
- Ability to solve problems and act as a mediator.
- Must be detail-oriented; can meet schedules and complete work assignments successfully.
- Must have strong computer skills, including word processing and database management.
- Valid Hawaii driver’s license and access to an insured vehicle are required.
Scheduling requirements:
General business hours are 8:00 a.m. - to 4:30 p.m. Monday through Friday, but this 40-hour-a-week, exempt position offers flexible scheduling to meet client and program needs. There may be times when you are required to work early mornings, evenings and weekends.
