Job Description

Required Qualifications

  • A Nursing Degree from an accredited college or university.
  • Current unrestricted license as a Registered Nurse (RN) in Hawaii.
  • A minimum of 1 year of experience as a supervisor in a hospital, nursing home, skilled nursing facility, or other related health care facility.
  • Knowledge and experience with skilled nursing and long-term care laws, practices, guidelines, and regulations.
  • Knowledge of nursing and medical practices and procedures.
  • Current Infection Preventionist Certification (or completion within 3 months of hire)
  • Current BLS/CPR certification.

Preferred Qualifications

  • 2 years of experience working as a Registered Nurse in a skilled nursing facility.
  • 1 year experience an Infection Preventionist.

Major Duties and Responsibilities

  • Assists the Director of Nursing in planning, developing, organizing, implementing, evaluating and directing the day-to-day operations of the Nursing department, along with its programs and activities, in accordance with current state and federal laws and regulations.
  • Interprets, communicates, and implements facility’s policies and procedures to nursing staff and monitors staff practices, performance, and implementation.
  • Participates in all admission decisions and procedures, as indicated.
  • Participates in the facility’s management and interdisciplinary team meetings to discuss census changes, resident conditions, regulations and requirements, and complaints or concerns.
  • Participates in the facility’s grievance program.
  • Reviews nursing documentation, physician orders, assessments, evaluations, consultant reports, department forms and ensures compliance, carryout recommendations and orders and indicated.
  • Evaluates the work performance of all nursing department personnel by conducting rounds, observing nursing care, treatments, and procedures.  Provides guidance, correction, training and education (in-services) as appropriate, to ensure nursing staff in competent in current knowledge and skills.  Participates in discipline according to operational policies, as directed.
  • Actively participates in the facility’s Quality Assurance Performance Improvement (QAPI) program.
  • Participates in the development, review, revisions, and implementation of the facility assessment, as needed and instructed.
  • Facilitates, serves, attends or participates in various committees of the facility as appointed.
  • Provides nursing staff supervision to ensure the delivery of compassionate quality care as evidenced by the delivery of care defined in the resident’s care plan and activities of daily living (ADL) needs to maintain optimal resident functions.
  • Ensures general cleanliness.
  • Collaborates with members of the interdisciplinary team, physicians, consultants, and community agencies to identify and resolve issues and improve the quality of services.
  • Assists with oversight of nursing schedules to ensure resident needs, regulatory and budget standards are met.  Covers open shifts as needed or directed.
  • Participates in the recruitment, hiring and termination of nursing personnel as indicated.
  • Promotes teamwork, mutual respect, and effective communication.
  • Participates in the preparation of inspection surveys, including but not limited to, conducting staff in-services, facility audits, participation in mock surveys, conducting observations, understanding state and federal regulations, as indicated.
  • Participates in the facility’s plan of correction response to an inspection survey and implements any follow-up required for nursing allegations.
  • Conducts resident assessments, evaluations, and treatments.  Develops, reviews, and revises resident care plans as needed.
  • Communicates directly with residents, medical and nursing staff, family members, department heads and members of the interdisciplinary team to coordinate care and services and respond to and resolve complaints and concerns.  Participates in resident care conference meetings.
  • Assists in oversight of resident incidents and concerns daily to identify any unusual occurrences and reports them promptly to the Director of Nursing, Administrator, and/or state agency for appropriate action.  Completes incident investigations to include identification of the root cause(s), prepare corrective action, and develop resolution.
  • Monitors for allegations of potential abuse or neglect, or misappropriation of resident property and participates in the investigative process.
  • Stays current with local, state, and federal regulations.
  • Attends webinars, in-service sessions, conferences, and training programs as guided.
  • Assists the Director of Nursing to ensure staff meet and stay current with conditions of employment in accordance with facility policies, state and federal regulations.
  • Conducts nursing department staff performance reviews and evaluations.
  • Monitors, assists and implements the infection control program in accordance with current infection control guidelines to prevent the development and transmission of disease and infection.
  • Tracks resident and staff immunizations, reports to required agencies, Department of Health, and/or CMS as required.
  • Monitors and tracks facility infections, both resident and staff, providing guidance on the facility’s infection control protocols.  Reports to local, state, and federal agencies as required.
  • Promotes safe work practices, safety rules, and accident prevention procedures to prevent employee injury and illness.
  • Fulfill all other functions as directed.
  • May provide direct nursing care as necessary.
  • Participates in facility events, activities, resident activities and special occasion events and needed.
  • Acts in an administrative/management capacity in the absence of the Director of Nursing and/or Administrator.
