** Hybrid Work Environment - Must reside in Hawaii **

 

Pay Range: $47,500 - $88,000

Note: Individuals typically begin between the minimum to middle of the pay range

 

The Community Health Worker (CHW) serves a critical role in advancing health and wellness within the community by fostering trust, advocacy for individuals and families, and facilitating timely access to appropriate care. The role performs outreach, offer educational resources, coordinate care, supporting closing care gaps, and connect members with relevant community services to address social determinants of health (SDOH). Collaborates with healthcare and community professionals to optimize health outcomes, member satisfaction, and overall community well-being. Participates as a member of the interdisciplinary care team (ICT).

  1. Identifies and supports the culturally relevant, social determinants of health (SDOH) needs of high-cost, complex members as they transition through various healthcare settings.
  2. Serves a critical role in advancing health and wellness within the community by fostering trust, advocacy for individuals and families, and facilitating timely access to appropriate care.
  3. Performs outreach, offer educational resources, coordinate care, supporting closing care gaps, and connect members with relevant community services to address social determinants of health (SDOH).
  4. Collaborates with healthcare and community professionals to optimize health outcomes, member satisfaction, and overall community well-being.
  5. Assists with and/or reviews comprehensive assessments completed by the RN--including physical, psychosocial, and health literacy evaluations, and related care plans.
  6. Ensure timely and through documentation of all care activities per federal, state, and NCQA requirements.
  7. Reports findings, concerns, and significant developments.
  8. Engages in quality improvement initiatives.
  9. Analyzes dashboard data for program enhancement.

 

Minimum Qualifications

  1. Associate's degree and three years relevant healthcare/social services experience; or equivalent education/work experience.
  2. Fieldwork experience such as home visits, conducting health screenings in the community, and/or navigating social services (like food, housing).
  3. Excellent communication skills, both written and verbal.
  4. Basic working knowledge of Microsoft Office applications, including but not limited to Word, Excel, and Outlook.
  5. Must have reliable transportation for in-person meetings with members at home or other locations.

 

Duties and Responsibilities

  1. Manage community health by establishing and maintaining trusting relationships with individuals, families, and providers to promote health, recovery, resiliency, and wellness. Advocate and support members and their families by prioritizing their needs and preferences, making sure they receive appropriate care and services promptly.
  2. Coordinate and collaborate with licensed clinicians and/or Health Management programs, as needed. Adjust workflows when necessary to provide optimal care for each situation or individual.
  3. Uses effective communication techniques, including motivational interviewing, to encourage members to attend regular provider appointments, close open care gaps, and utilize medical services that support their health. Engage with members and their families to discuss major health concerns and explore solutions to obstacles impacting service delivery, member satisfaction, cost, and community health. These efforts contribute to better care coordination for members overall.
  4. Delivers comprehensive education, resources, referrals, and connections to health-related services within the community. Facilitates the introduction of new or underutilized programs and services to community members.
  5. Provides guidance to community members facing complex health cases in navigating healthcare systems. Responsibilities may include, but are not limited to, conducting home visits, accompanying individuals to appointments, and supporting community events as needed. Exercises sound professional judgment and adhere strictly to scope and licensure boundaries to ensure actions are taken in the best interest of the community member(s).
  6. Provides culturally appropriate health education and instructions on using existing health and social services, presenting information clearly and effectively.
  7. Performs all other miscellaneous responsibilities and duties as assigned or directed.
