Job Overview
We are seeking a compassionate and skilled Licensed Practical Nurse (LPN) to join our healthcare team. Duties

• Conduct triage for incoming patients and manage acute care situations.
• Maintain accurate medical documentation in compliance with HIPAA regulations.
• Collaborate with interdisciplinary teams to ensure comprehensive patient care.
• Educate patients and families on health management and disease prevention.
• Support patients with developmental disabilities, Alzheimer’s care, memory care, and senior care needs.

Experience
Candidates should possess experience or knowledge in the following areas:

• Long-term care experience in nursing homes or assisted living facilities
• Knowledge of ICD-9 and ICD-10 coding for accurate medical documentation
• Familiarity with hospice care, acute pain management, and infection control protocols
• Experience in home care settings or community health initiatives
• Proficiency in basic life support (BLS) and phlebotomy skills
