Job Description

POSITION SUMMARY

The Wrap Navigator helps navigate a compassionate, efficient, productive, respectful, and safe Wrap meeting for the youth, family, and service providers.  Provides continued support to the Facilitator throughout the duration of the Wrap case. The Navigator assists the team in assuring strategies and solutions can be accomplished, if there are barriers, requests will be made to the Wrap Coordinating Committee.  The Navigator ensures that all confirmed key participants have a general but clear understanding of the Wrap process and protocols, makes final preparations and adjustments in collaboration and consultation with the Wrap Supervisor and Wrap Manager.

ESSENTIAL DUTIES AND RESPONSIBLITIES:

  • Responsible for working in a collaborative environment, with a team of Recorders, Facilitators, Scheduler, Administrative staff, Lead Navigator, and Wrap Manager. Collaborate as needed with all other programs statewide. 
  • Responsible for collecting, organizing, and analyzing all pre-Wrap documentation and files prior to each Wrap meeting.
  • Responsible for thorough, compassionate, ethical, efficient, and professional in-person and/or telephone interviewing of all key participants including youth, family, and professional sources to appraise strengths, concerns, and issues pertaining to the meeting.
  • Work collaboratively with Wrap Facilitator, Parent Partner and Youth Partner in a shared effort to engage the family, youth, and providers.
  • Responsible for timely preparation of calls, documentation, and database input regarding all assignments.
  • Responsible for the intake of new referrals. This includes, case mining, creating the file, uploading the referral information to the database, and notifications of referral assignment.
  • Attends all Wrap meetings and collaborates with the team to brainstorm individualized strategies to address needs and concerns of the family and youth.
  • Responsible for timely communication with team, including youth, family, agency and service providers to follow up on the completion of everyone have assigned tasks before the next meeting.
  • Responsible to meet and consult with the Facilitator before every meeting to share updates on team’s progress on tasks and other pertinent information.
  • Assist in gathering and sharing information and resources to the family, youth, and team as discussed in individual meetings.
  • Assists in identifying and addressing barriers that may occur in the Wrap plan, which includes consulting with Facilitator, Lead Navigator, and Wrap Manager.
  • Assists Facilitator in Wrap requests to the Wrap Coordinating Committee.
  • Responsible to assist Facilitator in confirming the meeting dates and locations with the youth, parents, family, and service providers one-two (1-2) days before a conference.
  • Responsible for organizing and updating case summaries.
  • Responsible for providing outreach service as needed regarding but not limited to Wrap and EPIC services in-person information to family and or other key participants who may not be reached otherwise.
  • Responsible for recording meetings and writing the report. Wrap report must be submitted to the Facilitator within three (3) days of the completion of the meeting. 
  • Appropriately seek peer mentoring as needed from other Wrap Navigators as well as daily supervision, mentoring, and coaching, from the Lead Navigator and Wrap Manager.

OTHER DUTIES/FUNCTIONS

  • Other duties, as assigned.
  • Wrap Manager will be available for consult and support on all above duties.
  • Attends and participates at trainings, all staff meetings, and individual supervision, as scheduled.
  • Recommends improvements and/or enhancements of job processes and work environment to ensure high quality levels of service deliverables plus timely addresses appropriate needs with Wrap Manager.

MINIMUM QUALIFICATION REQUIREMENTS

  • Must have a Bachelor’s degree from an accredited institution of higher learning or one (1) year related experience and/or training
  • Must be able to maintain an ethical, responsible, and professional attitude and demeanor under all circumstances in person, by phone, e-mail, and correspondence, in order to effectively and efficiently engage and maintain working relationships with families, professionals, who are undergoing complex and crises situations.
  • Current First Aid and CPR certification, training will be provided.
  • Valid driver’s license, acceptable motor vehicle abstract2, and access to an insured, reliable vehicle is needed.
PREFERRED QUALIFICATIONS
  • Individual with experience working with families whose children have been harmed or threatened with harm and in facilitation and/or mediation, preferred.
  • Individual with culturally relevant training and/or experience specific to the community served, preferred.

 

Pay: 

  • $19 to $22 per hour, commensurate with experience
