Job Description

The Outreach Worker/Kupuna Wellness Center Instructor position is responsible for outreach activities on the Island of Oahu which promote the organization, LMOW (Lanakila Meals On Wheels) program, and services to ensure outreach goals are met, with emphasis on seeking out and identifying potential participants and supporters.  This position may also assist with the operation of the Kupuna Wellness Centers as a program instructor and is responsible for facilitating activities for program participants which are inline with Lanakila Pacific’s values and program standards.

KEY SUPPORT RESPONSIBILITIES:

Plan and conduct daily outreach activities to promote the organization, LMOW program, and services.  Activities may include speaking events, resource fairs, and soliciting support such as referrals, donations, and volunteers from service and kupuna groups. 

Conduct timely follow up on all incoming referrals and individuals seeking information.  Outreach activities must meet program and contract goals/outcomes.

Seek out potential participants, caregivers, and referral sources with the purpose of increasing the number of program referrals and participants.  Screen and assess potential participants to determine preliminary program eligibility.  Make recommendations and assist with referrals to other services as needed.  

Complete intake and assessment for potential participants as appropriate to determine if participants are eligible and enroll them for program services.  Complete reassessments for ongoing program participants, and their home environment and living conditions, once a year or as needed, to determine if participants continue to be eligible for program services.   Communicate to applicants and/or their designated representatives regarding eligibility status and make recommendations and assist with referrals to other services if ineligible for program. Also identify if the participants’ needs have changed and other services may be needed and make recommendations and assist with referrals to other services as needed.

Accurately and timely document required outreach and participant information and outcomes in program paperwork, database, and reports. 

Provide coverage for the Kupuna Wellness Centers.  Responsible for the day to day operation of the center including overseeing approved volunteers, program participants, and guests. 

Schedule, coordinate, and conduct appropriate social, recreational, educational, and physical activities to ensure the Nutrition Education and Recreation goals are met.  Activies to include community guest speakers, service projects, health and public awareness.  Activies must consider the interests and abilities of the participants and support their cognitive, physical and social wellbeing in a healthful and respectful environment.

Accurate and timely completion of program reports including meal orders, program services, attendance, and food distribution (or waste).  Write daily summaries, ensure accuracy of project income, and incident/accident reports.

Develop and maintain positive rapport with program participants. 

Effectively communicate with LMOW staff and funding sources, including any updates or changes in participant eligibility status, emergency requests, or other service issues.

 

KEY SELECTION CRITERIA:

Required:

Education: High School Diploma or equivalent

Experience: Three (3) years of experience in a community-based program or related work experience.

Clearance/Certification:

1) TB clearance; 2) Criminal Background Check; 3) Child Protective Service/Adult Protective Service check; 4) Must maintain valid driver’s license and clean driver’s abstract (level 1 driver)

 

Preferred:

Education: Bachelor’s degree from an accredited institution in social work, psychology, education, marketing, or other related field. 

Experience: Minimum of one year experience providing direct services to kupuna in a community-based human service program.

Experience with program planning and coordination.

 

Working Conditions/Hours:

Monday through Friday; indoors and outdoors; regular travel to other program sites, meeting sites, outreach locations, etc.; required to work outside of normal business hours with advance notice.
