Job Description
To plan and conduct outreaches related to SNAP (Supplemental Nutrition Assistance Program), SNAP Nutrition and education components of SNAP benefits. To assist in developing outreach and educational materials for potential and current SNAP recipients. This project was supported by a Contract from the Department of Human Services, State of Hawaii. ESSENTIAL DUTIES & RESPONSIBILITIES:
- Attends outreaches and community events in a timely and consistent manner.
- Assists, research, and follows-up on clients and community partners questions, concerns, and comments.
- Be knowledgeable about SNAP and its outreach activities, including but not limited to eligibility requirements, application process, and properly respond to requests for information regarding SNAP.
- Administer the delivery of direct client services through SNAP Outreach, including assisting and submitting applications for clients and completing client contact logs and follow ups.
- Conduct pre-screening, client intake, and provide community referrals (as needed) for individuals interested in applying for SNAP benefits in office and at outreach sites and community events.
- Reviews income and expense information submitted with applications for SNAP assistance.
- Maintains records for the SNAP Outreach program including detailed client contact logs and an intake tracking spreadsheet for the status of current and pending SNAP applications.
- Maintains contacts with clients with limited mobility and assist them with SNAP related issues by conducting regular meetings and visitation i.e. home visits to the elderly and disabled, visits to homeless shelters and drop-in centers, etc.
- Maintains good relationships and work collaboratively with the Department of Human Services (DHS) staff and other service providers and acts as a client-DHS liaison in facilitating the application process, i.e. couriering application or supplemental documents to DHS on clients' behalf as necessary.
- Assist program manager with preparing periodic written and or verbal reports regarding outreach efforts for the President & CEO, the Department of Human Services, and/or other funders and supporters as requested.
- Assist program manager with maintaining data tracking mechanisms for the SNAP Outreach Program in compliance with all DHS and agency reporting requirements.
- Be knowledgeable about other public benefits that SNAP applicants may be eligible for and be able to share that information during SNAP Outreach and intake process.
- Plan outreach, participate in community events, and attend public forums and presentations for the promotion and education of SNAP.
QUALIFICATIONS:
- Bachelor's Degree in Communication, Psychology, Human Services or related field and one (1) year of customer service related experience, preferably in the Human Services environment.
- An exception may be made for the above-mentioned educational requirements, on an individual case-by-case basis, if the candidate meets the following combination of required minimum education and experience:
- HS diploma or equivalent and two (2) years of job-related experience where knowledge and abilities have been demonstrated , preferably in the Human Service environment.
- Extensive working knowledge of computer programs, including but not limited to, Microsoft Word, Excel, Power Point and other database operational software.
- Ability to work directly and collaboratively with diver persons and programs found in a multi-faceted agency and community.
- Ability to organize, direct, review and evaluate activities and volunteers.
- Effective communication skills (listening, speaking, and writing) with proven ability to take and relay detailed and accurate information.
- Detail oriented and ability to multi-task.
- Valid Driver's license, clear traffic abstract, has access and is willing to use properly insured vehicle.
- Current TB Clearance.
- Must be willing to work occasional Saturdays.
