We are seeking a compassionate and experienced Social Worker with a strong mental health background to join our interdisciplinary team and make a meaningful impact in the lives of our residents and their families.

The Social Worker plays a vital role in ensuring residents receive comprehensive psychosocial support, with a particular focus on mental health needs, behavioral support, and emotional well-being. This position partners closely with nursing, medical providers, families, and community resources to create individualized care plans that promote dignity, stability, and quality of life.

• Conduct psychosocial assessments with emphasis on mental health history, current functioning, and behavioral needs
• Develop and implement individualized care plans addressing emotional, social, and mental health needs
• Provide supportive counseling and crisis intervention for residents experiencing behavioral or emotional challenges
• Collaborate with interdisciplinary team members to support behavior management strategies and care coordination
• Serve as a liaison between residents, families, and external mental health providers and community resources
• Assist with discharge planning, ensuring continuity of care and appropriate mental health follow-up services
• Advocate for resident rights, dignity, and culturally competent care aligned with the Aloha Spirit
• Maintain accurate and timely documentation in compliance with state and federal regulations

• Bachelor’s or Master’s Degree in Social Work, Psychology, or related field
• Prior experience in mental health, behavioral health, or psychiatric settings strongly preferred
• Experience in long-term care or skilled nursing is a plus
• Knowledge of psychosocial assessment, crisis intervention, and behavior management techniques
• Strong communication, empathy, and problem-solving skills
• Ability to work collaboratively in a fast-paced, team-oriented environment
