Social Worker - Healthcare

We are seeking a compassionate and dedicated Social Worker to join our healthcare team. In this role, you will provide crucial support to patients and their families, helping them navigate complex medical and social challenges. Your expertise will be instrumental in improving patient outcomes and enhancing the overall quality of care in our healthcare facility.

Responsibilities:

- Conduct comprehensive psychosocial assessments of patients and their families
- Develop and implement individualized care plans to address patients' social and emotional needs
- Collaborate with interdisciplinary healthcare teams to ensure holistic patient care
- Provide crisis intervention and support for patients and families facing difficult medical situations
- Connect patients with community resources and services to support their ongoing care and recovery
- Advocate for patients' rights and needs within the healthcare system
- Maintain accurate and up-to-date documentation of patient interactions and interventions
- Participate in case conferences and team meetings to discuss patient progress and treatment plans
- Provide counseling and emotional support to patients and families coping with illness or loss
- Assist in discharge planning and coordination of post-hospital care

Requirements:

- Strongly encouraged Bachelor of Arts in Healthcare or Social Work
- Minimum of 2 years of experience in healthcare or social work
- Strong knowledge of medical terminology and healthcare systems
- Excellent interpersonal and communication skills
- Ability to work effectively in a fast-paced, multidisciplinary environment
- Proficiency in electronic health record systems and Microsoft Office Suite
- Demonstrated ability to handle sensitive and confidential information with discretion
- Fluency in English; additional language skills are a plus
