Job Description

The TLC Coordinator ensures quality services are provided to the TLC program participants and in compliance with Department of Health (DOH) Waiver Standards, coordinating services with participant’s circle of support to assist participants in acquiring, retaining, and improving their skills to achieve their desired outcomes and goals.  The TLC Coordinator manages and supervises staff to ensure organizational and program objectives are carried out, and policies and procedures are adhered to.

 

KEY SUPPORT RESPONSIBILITIES:

Position Specific Duties:

  • Coordinates with TLC staff and participants’ circle of support in developing and implementing the participant’s Individualized Plan (IP) in conjunction with the Individualized Service Plan (ISP).

  • Writes and implements the care plans to track participants’ program areas and ensures that these plans are being implemented by TLC Instructors. 

  • Ensures that all reports satisfy the total reporting requirements of the program.

  • Assists participants with Activities of Daily Living (ADLs), including toileting, hygiene and grooming, dressing, feeding, eating, etc.

  • Provide transportation through the TLC transit program as needed.

  • Ensure confidentiality of participant records.


    Financial:

  • Completes and monitors budget for community outings, program supports and/or program supplies.

  • Monitors individual supports budgets for TLC participants.

     

    Human Resources/Supervisory:

  • Supervise TLC Instructors that are assigned to their participant group and serve as a backup supervisor if other Coordinator(s) are out.

  • Provide coaching and leadership to TLC Instructors to ensure that organizational and program objectives are met.

  • Appropriately address and document incidents and performance issues, involving the TLC Manager and/or Director of TLC as needed.

  • Ensure TLC Instructors comply with company and departmental policies and procedures.

  • Complete Employee Performance Evaluations (EPEs) for their assigned TLC Instructors according to their review period.

  • Provide new hire training for TLC Instructors in compliance with DOH Waiver Standards as well as ongoing training as needed or required.


 

KEY SELECTION CRITERIA:

Required:

  • Education: Bachelor of Arts (BA) in Education or Social Sciences or BA in another field with one year documented experience working with ID/DD or the elderly population.

  • Experience: A minimum of one year of progressively responsible work experience teaching, coaching or facilitating, and working with individuals with a variety of disabilities or similar populations.

  • Clearance/Certification: Must maintain the following clearances/certification: 1) annual TB clearance; 2) current First Aid and CPR Certification; 3) Criminal Background check; 4) Child Protective Service check; 5) Adult Protective Service check; 6) Fieldrprint Fingerprinting and 7) List of Excluded Individuals and Entities (LEIE).

  • Valid driver’s license and clean driving abstract with access to an insured vehicle.
