Job Description

The Teaching and Learning Center (TLC) Coordinator ensures quality services are provided to the TLC program participants and in compliance with Department of Health (DOH) Waiver Standards, coordinating services with participant’s circle of support to assist participants in acquiring, retaining, and improving their skills to achieve their desired outcomes and goals. The TLC Coordinator manages and oversees all aspects of program participants care assigned to them, including training of staff, overseeing job performance of direct care staff to ensure program objectives are carried out and service delivery is provided in a safe and effective manner, responding to day to day as well as urgent matters, documentation, and chart maintenance.  The TLC Coordinator supports the mission, vision, and values of the organization, upholds, and enforces the guidelines and policies of the organization and demonstrates Lanakila Pacific’s values of teamwork, excellence, achievement, and motivation in all personal interactions and exemplifies Lanakila’s STAR service standards.

 

KEY SUPPORT RESPONSIBILITIES: 

Position Specific Duties:

  • Coordinates with TLC staff and participants’ circle of support in developing and implementing the participant’s Individualized Plan (IP) in conjunction with the Individualized Service Plan (ISP).
  • Writes and implements the care plans to track participants’ program areas and ensures that these plans are being implemented by TLC Instructors. 
  • Ensures that all reports satisfy the total reporting requirements of the program.
  • Assists participants with ADLs (Activities of Daily Living) including toileting, hygiene and grooming, dressing, feeding, eating, etc.
  • Provides direct care as needed, including telehealth services.
  • Provides transportation through the TLC transit program as needed.
  • Ensures confidentiality of participant records.
  • Completes and monitors budget for community outings, program supports and/or program supplies.
  • Monitors individual supports budgets for TLC participants.
  • Provides coaching and leadership to TLC Instructors to ensure that organizational and program objectives are met.
  • Provides new hire training for TLC Instructors in compliance with DOH Waiver Standards as well as ongoing training as needed or required.
  • Assists in the removal of all fire and safety hazards.
  • Know location of all fire extinguishers in the area and how to effectively use them. Ensures that first aid kits are available, and that TLC staff are aware of their location. Knows how to properly apply first aid. Ensures first aid kits are replenished as needed. 
  • Know or research how to address participants’ physical and/or mental emergencies and trains TLC Instructors.
  • Ensures facility is operational during business hours and secured at the end of each business day.
  • Serves as a backup supervisor if other Coordinator(s) are out.
  • Appropriately addresses and documents incidents and TLC Instructor performance issues. Ensures TLC Instructors comply with company and departmental policies and procedures.
  • Ensures participant files meet Department of Health (DOH) Waiver Standards and comply with all timelines set for completing Quarterly Reports, monthly Supervisory Visits, and Care Plan’s.
  • Maintains service statistics.
  • Uses Therap Electronic Documentation to maintain participant records.
  • Provides back-up administrative assistant support as needed, including answering phones, lunch counts, follow up for participants who fail to come in, etc.
  • Oversees and directs the day-to-day work of staff, assists with performance evaluations and improvement plans as needed, and appropriately addresses complaints, documents incidents and disciplines employees.
  • Communicates effectively with all clients, programs/departments and staff, both orally and in writing; read and understand written material and write effectively to meet the job requirements.

WORKING CONDITIONS/HOURS:

  • Scheduled to work 5 days a week. Monday through Friday; 7:30am-4:30pm. 8-hour shift may vary. May be required to work outside of normal business hours, including weekends and holidays to meet operational needs and contract requirements.
  • Indoors, classroom/office setting, outdoors; travel to other program sites, meeting sites, etc., as required.

MINIMUM QUALIFICATIONS:Required

  • Education: Bachelor of Arts (BA) in Education or Social Sciences or BA in another field with one-year documented experience working with ID/DD or the elderly population.
  • Experience: Minimum of one year of progressively responsible work experience teaching, coaching, or facilitating, and working with individuals with a variety of disabilities or similar populations.
  • Clearance/Certification: Must maintain the following clearances/certification: 1) annual TB clearance, 2) current First Aid and CPR Certification (Obtain on the job), 3) Criminal Background check, 4) Pre-employment drug and alcohol testing. 5) Child Protective Service check/Adult Protective Service check 6) Fieldprint Fingerprinting, 7) List of Excluded Individuals and Entities (LEIE).
  • Valid driver’s license and clean driving abstract with access to an insured vehicle.
