Social Services Specialist (223623-14)
We are seeking a compassionate social work professional dedicated to supporting patients and families through care coordination, advocacy, resource navigation, and community-based services.
Hours
Full-time, Monday-Friday, 7:45am-4:45pm
Location
Honolulu, Oahu
Pay
$28 to $31 an hour
Responsibilities
- Connect patients and families with community resources, financial assistance programs, and support services.
- Arrange transportation, medical appointments, facility transfers, and discharge plans.
- Conduct assessments of discharge evaluations to ensure appropriate care placement.
- Coordinate referrals and collaborate with healthcare providers, community agencies, and support organizations.
- Provide guidance and support to patients and families during medical crises, end-of-life situations, and bereavement planning.
- Assist with legal, financial, and administrative matters, including advance directives, benefits, and required documentation.
- Develop care plans and participate in interdisciplinary case conferences.
- Maintain accurate patient records, case notes, and care documentation.
- Communicate regularly with families, healthcare teams, and community partners to support patient care.
- Facilitate caregiver support groups, community outreach efforts, and educational activities.
- Participate in team meetings, training programs, and other duties that support patient services.
Qualifications
- At least one year of professional social work experience.
- Healthcare or care management experience preferred.
- Ability to work with diverse populations and complex family situations.
- Knowledge of social service programs, community resources, and benefits systems.
- Experience maintaining documentation and managing multiple cases.
- Ability to make sound independent decisions while working collaboratively with a team.
- Valid driver's license required.
- Bachelor's Degree in Social Work required.
- Master's Degree in Social Work preferred.
