# HHP Outreach Worker

We are seeking a compassionate and dedicated HHP Outreach Worker to join our team on a part-time basis. ## Responsibilities

- Conduct outreach activities to identify and engage individuals and families who may benefit from program services
- Build and maintain trusting relationships with clients, community members, and partner organizations
- Assess client needs and provide appropriate referrals to internal programs and external community resources
- Maintain accurate and timely documentation of all client interactions, services provided, and outcomes achieved
- Participate in community events, health fairs, and other outreach initiatives to promote program awareness
- Collaborate with team members and community partners to coordinate comprehensive service delivery
- Follow up with clients to ensure successful connection to services and address any barriers to access
- Maintain confidentiality of all client information in accordance with organizational policies and applicable regulations
- Attend required staff meetings, trainings, and professional development opportunities
- Prepare regular reports on outreach activities, client contacts, and program outcomes
- Advocate for clients and help them navigate complex service systems
- Identify and report emerging community needs and trends to program leadership

## Requirements

- Minimum of one year of experience in outreach, case management, social services, or a related field
- Strong interpersonal and communication skills with the ability to engage diverse populations
- Demonstrated ability to work independently and manage time effectively
- Proficiency with computers and standard office software applications
- Ability to maintain professional boundaries while building rapport with clients
- Strong organizational skills and attention to detail
- Valid driver's license and reliable transportation for community-based outreach activities
- Ability to work flexible hours, including occasional evenings and weekends as needed
- Bilingual skills are highly desirable
- Knowledge of community resources and social service systems
- Ability to work effectively in a team environment
- Strong problem-solving skills and ability to think creatively
- Cultural competency and sensitivity to work with diverse populations
- Ability to maintain confidentiality and handle sensitive information appropriately
